Checklist Tasks opens to a blank page

Off Boarding Checklist Items do not appear on the Checklist Tasks page

I have created an off boarding exit survey questionnaire to trigger when an employee terminates. The the employee does see the notification card  and bell icon notification which will open the worklist and eventually get to the survey. However, when the employee clicks on the Current Tasks icon (under the Me or About Me section) the page opens to a blank page.

Does anyone know how this can be fixed? We are wanting the employee to be able to click on the Checklist Task icon and see all the tasks within the checklist.
