Error when attempting a file based import in data management

Hi Everyone,

I am attempting to import the attached csv file into planning. The source of the file is a custom OTBI query form the ERP that was exported as csv. I am using a Multi Column - Numeric Data file type. Upon importing, I get a generic error that I am unable to debug. Attached is the level 5 log of the import process, a sample of the csv file and the configuration options for the import format, location, etc.

Any suggestions for debugging this?
