Termination category blank - how to solve?

How to have termination category for active employees with termination as future record

Hi, 

 

we need to identify active employees that have a termination planned in the next month and the related termination category.

Business want to deliver an exit survey before the termination date to employees with voluntary terminations only.

I already implement a report, but termination category for active employees is blank. My report is attached. 

Any ideas?
