Error when converting Contingent Worker to Employee

When trying to convert a contingent worker to employee, HDL is throwing the error "You can't supply multiple data lines for the same record in the same file because the component {BUS_OBJ_NAME} isn't date-effective." What could be causing the issue ? I am using below worker.dat
