We have a custom disability form and I am making changes to the Disability Info page.

I have found most of the objects to hide or update, but there a few giving me trouble and I could use some help

Enable/Display Disclosure date on the Add page (we replaced all of the text the delivered form)

Header above Yes/No check boxes - I found text in the source code, but nothing I modify shows up

Country - ideally I would like to make it US only, but is there a way to reduce or reorder the list?
