Form to request Learning Catalog items

Does anyone have a form they use when someone is requesting Learning Catalog items?

We are going live with Learning in 2021, and I'm looking for a form of sorts that someone has created that will help a requestor think through how they want their course or curriculum structured in the LMS, e.g., will a specialization be needed to "house" multiple courses? Should the SCORM or videos be individual courses or as an activity within an offering? Do you have assessments and/or a need for evaluations? Is this vILT, ILT, blended, or self-paced? Are there PDFs to attach? Do you need to add a weblink for a synchronous learning platform?
