How to meet that requirement for a report?

Summary

Output format XLSX, 2 header rows and specific sheet name

Hi community,

I need to develop a report with these 3 requirements:

-Output format XLSX,

-2 header rows

-specific sheet name in the Excel

I developed an XLS (not X) template (attached) that covers requirements n. 2 and n.3 but not the n.1. (attached formats available)

I developed an XPT template that covers requirements n.1 but not n.3 but and n.2.

Do you have any idea how to reach all these requirements?
