How to load sample report in Oracle HCM

Hello experts,

I need to load a custom template as example in Oracle HCM. I want to load an image showing a sample performance document.

In addition, I have created a custom button in the employee performance document and when clicking on it, the custom image I have loaded as example should appear. I have tried to load in Oracle Reporting and Analytics page a pdf document but it doesn't work.

How can I load the sample image to connect to the custom button?
