Create a document that will populate certain fields directly into the document

Looking to create a document i.e. VOE that will grab and populate the data built into the fields requested.  Is there a way to do this out of OTBI?  For example an employee wants a verification of employment, they would click on the document then all of the fields specific to that document would populate with the associate's info, then they can export and save.  I am thinking that I would need to create a template? 

Any help would be greatly appreciated.
