Leavers Performance Rating

Hi,

 

We are looking to build a report which captures the performance rating for leavers, so we can add extra depth to our turnover reporting, however when we try to pull the performance rating through, all historic ratings are displayed. 

Has anybody successfully created a similar report that they could share with me?
