ORC Error FADomain.sever.instance: uiserver_as3_01

Hello any help on an ORC error please:

I have tried looking on Oracle HELP pages but they dont have an error that matches _as3_01

The error is FADomain.server.instance: uiserver_as3_01

So while filling out Job Req's we have gotten the above error more than 2 times but the requisition does get filled & candidate is moved to HR and that task is completed.

But we do not have any indication what the Error is
