How can we add a new Tile under Personal Information Tile

We would like to add a new Tile under Me > Personal Information page. so associates can add their COVID vaccination information and this should be part of Personal information change flow and also should go to approval as normal personal info changes.

May be something like below,

section 1 - COVID vaccination status

Fields - Vaccinated (LOV - Yes/No), Vaccination Date (Calendar to choose)

section 2- Attachment

please guide how to achieve this.
