Person Legislative Attributes not all displaying

Summary:

I have created a number of customer defined legislative flexfields and enabled in TDS. However, only one of them is displaying in the application form.

Content (required):

I have created a 4 cPER_PERSON_LEGISLATIVE_DFF's and enabled in TDS using the candidate diversity rule. I've also enabled a number of OOTB diversity fields. All of the OOTB ones are displaying in the application form however, only one of custom fields is displaying. Does anyone else have experience of this?
