Need link for questionnaire template and subscriber for Learn

Hello everyone,


I need to find questionnaire template and the subscriber it is linked to. I was able to find 4 tables related to questionnaire and subscriber. I want to know which template belongs to which subscriber but I am unable to find linkage between them.

Content (required):

I tried using module_id and business_group_id but it doesn't help. Below are the tables which I found.

For questionnaire template - HRQ_QSTNR_TEMPLATES_TL, HRQ_QSTNR_TEMPLATES_B

For Subscribers - HRQ_SUBSCRIBERS_TL ,HRQ_SUBSCRIBERS_B

Is there a way we can find the subscriber which is consumed by questionnaire templates?
