Answer options are getting jumbled in the Learning Evaluation Questionnaire. How to resolve this?

Summary:

Dear Team,

Answer options are getting jumbled in the Learning Evaluation Questionnaire

Navigation : My Client Groups -> Questions -> Created a question with single choice and with 3 responses. And selected "Vertical" in Response order also.

Added that Question in Questionnaire Template.

Created a Questionnaire basing on that Template.

Added that Learning Evaluation Questionnaire in My Client Groups -> Learning -> Configure Evaluations

Now I enrolled into a course-> Completed the assessment-> The I clicked on Evaluate button-> Now Learning Evaluation Questionnaire has been opened.

Now I can see the answer options for the questions got jumbled.

Though I selected Response Order as Vertical why this jumbling is happening.
