Questionnaire template for HR

Hello Team,

We have received one requirement where after employee resignation , when HR has discussion with resigned employee, HR should be able to maintain and save it somewhere.

Currently we don’t track employees HR in the system anywhere like how we do Line managers in the system

Is this possible to achieve via any form creation with the help of Questionnaire which can be shown to only HR?
