Is there a way to add a questionnaire into a job application form?

I am creating an application form for on-call firefighters and would like to add a questionnaire into the application form, which asks them questions about availability, how long it takes to get to the station etc. I could do it with Pre-screen type questions but it makes the application look a bit messy so was wondering if you can add a questionnaire. But I want it as part of the application that the manager can then see when they shortlist. So I dont want the form to generate once the have submitted the application but be
