Custom Field shows in Template view but no when creating a live Requisition

We created a user defined field to include our EEO disclaimer information for sites located in the US. The field was added to the user defined form for the requisitions. It shows correctly in the "Template View" and appears to function as desired. However, when the recruiters use the template to create a new req, the EEO field vanishes. 

I'm sure I'm missing something simple, but I can't figure out what's causing that behavior. Any suggestions on what I should check? I've gone through all of the documentation, and don't see anything amiss.
