How do we make the Action Reason Code Required (mandatory) on all Action Type and Action Code?

Client would like to use this field for reporting purpose and would like to make this field required on all actions. Has anyone done this before? Is there any issues with making this required? Is there any downstream impacts we need to evaluate and take into consideration?
