Address fields Unique Identifier required for Poland

Summary:

We require the Unique Identifier for each of the address fields, like code for Address Line 1, Address Line 2 etc and also the Unique Identifier for the values against each field for Integration.

Content (required):

We have built report for Address Query. However we are getting each field details. We need to have Unique Identifier for each of the field details such as code for Address Line 1, Address Line 2, Postal Code, City, Province which is referred to as Municipality, County which is referred to as Community and State which is referred to as County.

We also want to have the Unique Identifier for the values which are coming under each of the Address fields mentioned below.
