OCI: System Fails To Mount Attached Block Volumes As It Is Unable To Load The "iscsi_tcp" module

SYMPTOM

Additional mount points - /u01 and /u02 are not mounted after a reboot.


CAUSE

After performing a “yum update” and rebooting, the system fails to mount the attached block volumes, the following messages are seen in the logs:

$ grep iscsi /var/log/messages
Aug 10 10:37:09 hostname iscsid: libkmod: kmod_module_insert_module: could not find module by name='iscsi_tcp'
Aug 10 10:37:09 hostname iscsid: iscsid: Could not insert module tcp. Kmod error -38
Aug 10 10:37:09 hostname iscsiadm: iscsiadm: Could not login to [iface: default, target: 
iqn.2015-12.com.oracleiaas:ee96767d-30a8-4d86-9d54-b197533f2f0e, portal: 169.254.2.3,3260].
