Customize Text wrt specific Document Type

Summary:

Client requires instructions to be displayed for a "Tuition Reimbursement Phase II" document type. The instructions shouldn't be displayed for any other document type.

Content (required):

A list of instructions each exceeding 150 characters should be listed as bullets. The value set PER_DOR_4000 isn't available and thus I need to add a custom field and display it for only this one document type
