Request for Information flow not triggered with successful return of fast formula condition

Summary:

We created a Request for Information flow in Candidate selection process in the 1st Phase & state of the CSP where we are using the fast formula to check if the job requisition Primary location is a city from Quebec. On checking the log for the fast formula, the condition evaluates to true and returns Y, however the action to "Request more Information" is not triggered. Without the fast formula condition the Request for information flow seems to work. We are running the "Perform Recruiting Candidate Selection Process Actions" each time the candidate has submitted their application.
