Home Address of another Employee , was added to the contact information of another Employee

Home Address of another Employee , was added to the contact information of another Employee and in the transaction console only the transaction of the employee whose Home address is changed is reflecting as submitted and not the employee who actually added. Would like to know suggestions on this.
