Approval issue for ICP's

Summary:

I have configured approvals for ICP's using BPM. The same approvals are working in lower environments but it is not working in one of the latest environment.

Can someone guide me, how to debug the approvals, as i checked the transaction in the transaction console, the request is getting by-passed and not able to rectify it.
