Restrict the action on changing working hours page

We are giving option to client to extend employee for 3 month.

The extension can happen in sequence like 1 month + 2month OR 1 + 1+ 1 or 3 month directly.

Anytime the 3 month count is reached, the page should block further actions Manager/HR.
