Approvals shows pending even when it is approved

Summary:

Hi Experts!

We just had an issue with approvals where the transaction shows successfull when entered by click into the transaction. But in the UI status bar it shows 'pending' and could terminate the transaction as well. I tried providing the privilege 'PER_APPROVE_TRANSACTION_PRIV' but still the result were th same. Does anybody have any idea on why it behaves like this. All answers are welcomed.
