Approval status is in progress but transaction is stuck with initiator

Hi,

We have a scenario where employee is not able to add/update any goals, when we checked it was because of pending approval transaction.

The transaction is pending for long time, the status shows as in progress but the transaction is stuck with initiator itself.

We have only one active rule to line manager (manager is active and approval fetched the supervisor hierarchy when checked with diagnostic report)

What is the root cause of this issue?
