Failure in transaction console for learning incident reports

Summary:

I've been informed by our systems team that we've had a number of failed learning incident reports show up in the transaction console over the past few weeks, with the error: "Potential invalid input data in the approval flow caused a system error in the orchestration flow."

Does this mean that someone has reported a course, but somewhere along the way, the workflow has failed? Learning administrators have not received an email notifications for these reports.

I just wondered what would cause this to happen and whether there is a fix?
