Can I make a leaver report as a graph showing reasons of termination?

Summary:

I have made a leaver graph in excel showing leaver reasons by month and have tried to recreate in Oracle, the leaver reasons are appearing, but not the correct count of that reason. (In June we had 13 assumed resignations) Is there a way to generate the count so that I can produce a bar chart? Any support would be very helpful.
