Total Submissions by Form Field Value

I am trying to create an Insight analysis in a format similar to the one below using the Form Submission Data subject area:

The filter is specifying the particular Form ID I am interested in along with the Form Field Name.

I am struggling to figure out how to create this, please can anyone help?
