Default Payroll attributes on Convert Pending worker page

Summary: On the Convert Pending Worker page in Payroll tab we have 3 fields (Overtime Period for Payroll, Tax Reporting Unit, Overtime Period for Assignment) which have only one value in the LOV. So User wants to default that value in the fields and make it as read only.
