The Document Record should not display unselected fields after submitted

The Dcoument Record should not display unselected fields after submitted.

We have created Consent Document type to capture the Employee details, In which we have a requirement to display only fields selected by employee while submitting the DOR. The Unselected fields should be hidden. please suggest any workaround to meet our requirement.
