How to create Approval rule based on Outcomes (APPROVE and REJECT)?

Summary:

Ex : There are three level of Approvers are there in sequential. Initiator initiates the transaction it will goes to the 1st approver, 1st approver Approves the task that is assigned after that it will goes to the 2nd approver and if the second approver REJECTS that task the notification will be sent to the 1st approver also. As of now who ever rejects the notification will be sent to Initiator we need for the 1st approver also. please suggest.
