Birthday Notification to be sent with Custom text on Image

We have requirement to send birthday notification to employee by keeping RM , HRRM , Employee direct reportee and RM direct reportee in CC
The content of email is :-
Dear < First Name >
< Image with custom text >
Custom text on image like
Wish you a very happy Birthday < Employee First Name >
Overall we need requirement to have Image in email with custom text on image.
Can we handle this requirement from Alert job Composser or from BI reports ?
