Can someone provide a sample of the health and safety incident submission confirmation email?

I am implementing the module and before allowing this email to be sent, we would like to see what it looks like from the end-user's perspective. Since we are not live with Health & Safety, we are using dummy email addresses and cannot test the confirmation. Does anyone have a sample screenshot they can provide with what the email looks like?
