Question - extra field on the employee terminate page

Summary:

I have a requirement from a client to add a extra field on the employee terminate page. I tried using a flex field with the dff code "PER_PPS_DF. However even after deployment the DFF did not work? Any idea why this might have happened, or is there some other DFF code that may need to be used?
