Duplicate AOR Approvers are visible in the Approval History

In the Request a Position Change approval, the names of AOR approvers are duplicating in the approval history. The approval process is working correctly. However, the issue lies with the duplication in the approval history.

Additionally, single AOR is assigned to each representative.

Does anybody know what could be the reason for this?
