Transaction stays in In progress status even after completion

Summary:

Hi Experts!,

We are experiencing an issue with approvals. The submitted 'Change Assignment' transaction was approved by the user, but it shows as pending or in progress status. Does anybody know the reason for this behaviour? We have been facing similar issue in prod, whentrying to replicate the same transaction in test, it seems to be working fine.
