duplicate direct reports, neither opens

I have a manager that has a direct report appearing two times in the My Team list. One of the lines the grey circle icon has the employee initials (like all the others), on the other the grey circle is blank. When selecting to view, neither opens - only a blank white screen. What is causing this?
