How to create Spreadsheet Loader to Load People Group in Assignment Page?

I am working on creating spreadsheet load to update People Group of a batch of colleagues. While I tried to follow steps from the guide , the load is unsuccessful.

I need to load the field for about 500+ employees and this may be an ongoing activity.

To create template, I followed below steps -
