Problems recording external learning

When a learner tries to record a piece of external learning, they fill in all of the details but when they click save they get the following error message, has anyone had this issue and if so what did you do to fix it please?

Your changes couldn't be submitted because of a server error. Try again later.Definition ContextCode of type Attribute is not found in LearnerLearningVO_assignmentDFF_LearnerLearningVOToLearningRequestFlexVOVL_LearningRequestDFFVO.

We have tried multiple times with different people and none of us can upload any external learning
