Has anyone managed to collect data on an internal application form for internal candidates referees?

Summary: Hi

We do not use a third party to take up references and pre-employment checks on candidates and so need to collect the data ourselves. Its our policy to have a reference for any internal candidate thats successful in getting a different role, does anyone have a way to collect the information about who the referee should be from the candidate? We have a solution for external candidates but nothing for internals, at the moment we have to send emails outside of the system to request the details etc and it would reduce so much time if we could
