RMI not triggered second time

Summary:

We have a CSP created we have 2 RMI. The configuration is as follow;

1 - at Initiate job offer phase the RMI is set it initiate check.

2- once candidate accepts the job offer we have RMI triggered at Offer Accepted state.

This set up worked for us at first instance but now it is not working
