Approval rules is failing and/or following wrong rule

Hello everyone,
The client has the following requirement for the "Change Personal Information" flows:
- If PWK initiates, then autoapprove
- If HR initiates, then autoapprove
- If EMP initiates, and the changes are in A,B,C (i.e., Person Name, Disability Info, Passport), then Representative approves
- If EMP initiates and the changes are in D,E (i.e., Phone Details, Email Details), then send FYI to representative and autoapprove
To do so, I have created 4 different rules for the above requirements. Please find the full rules in the attached document.
