Approvals are working fine but transaction log is showing fail

Summary:

Hi All,

We have a requirement to create a transaction approval for Request New Position with below 3 criteria's

  1. If Line Manager initiates > HR Specialist Approval > HR VP Approval
  2. HR Specialist Initiates > HRVP Approval
  3. HR Admin Initiates > Auto Approve

We were able to configure these flows and all 3 approvals are working fine as expected but the Rule Evaluation Results showing "FAIL" in the transaction diagnostics report.

We are not sure why the report shows fail when all the rules are working fine as expecting and routing correctly to the defined users.
