Change Personal Information for Address Change transaction is blank

Summary:

Hi Experts!,

We have been facing an issue with data not being populated correctly only for address change transactions, if we change name or demographic info or other personal information attributes it seems to be working fine. I have noticed couple of discussions happened on the same. Does anybody aware that oracle has provided with any workaround? Does coming up with a custom template for address change would that work?
