In 24 D Dialog box opened on action click shows incorrect action name. Is this product bug?

Summary:

We have recently upgraded to 24D. It is observed that, sometimes, the action name shown in the dialog box is not correct.

When the below dialog displayed, the action performed by the user is Save in the dialog; it is showing Cancel approvals. Similar behavior was observed intermittently for some other actions as well. Is this known bug?
