Add Contingent Worker flow error

Summary:

Hi All,

We are encountering a weird issue with Add Contingent Worker approvals. The transaction is approved by the recipient but in the console, it shows as Pending even after refreshing it multiple times. But if we terminate and resubmit it works fine

And the rule logic also follows the basic one IF (1==1) THEN Approval to the user A. We are getting this issue for multiple users now. We have also tried running the diagnostic reports for these users and not issues were found either.
