HCM Approval Delegation not working

Summary:

We are facing the situation where Approval Delegations have been set up for a manager, but the transactions are not being routed to the delegate. Instead they are still going to the original manager. The transaction initiator and delegate for the transaction is the same person. Does this matter?

Example: Manager A delegates to Manager B. Manager B submits a transaction for Employee X which still goes to Manager A, instead of being delegated to Manager B.
